On the CMS-1500 form, which is the correct way to bill:
- J0574 at 2 units, using the NDC number for the 8MG pill in box 24A on one charge line
- If this is the correct method, after the NDC number would you amend with a ME16 for total usage for the day or would you amend with a ME8 for the individual dose?
- J0575 (10+mg at 1 unit- rolling total milligrams administered for the day) and using the NDC number for the 8MG pill in box 24A
