Surgery Coding Help Needed

Greetings and Salutations from Iowa :o

I was asked to prior authorize some CPT codes for the following surgery and I'm completely at a loss for which codes to use.

Here is was the provider stated he was going to:

Endoscopic resection of supraglottic cancer with laser
bilateral selective neck dissection levels 2 through 4
biopsy a left palatine tonsil
