Hello everyone,
Checking to see if anyone has been submitting the TCM service CPT codes 99495, 99496 for Cardiology specialists?
On my research it says Specialists can provide these services. But I am unable to find further details except for this one from AAPC : https://www.aapc.com/blog/82202-transitional-care-management-time-to-get-it-right/: "For example, if a patient has a history of atrial fibrillation and congestive heart failure and they are admitted due to exacerbation of these conditions, TCM services following discharge would logically be provided by the patient’s cardiologist. This provider is best suited to provide comprehensive care and arrange the appropriate care model for these conditions".
We are thinking of implementing it and would like to hear your experience.
