I just started coding anesthesia visits for colonoscopy's and EGD's. I have a few questions some things just don't make sense to me. Lets say the patient is coming in for a Hx of polyps, found a polyp and hemorrhoids. My question is do you use K63.5 for the polyps found or do you actually go into the pathology reports to see if it was adenomatous polyp and use the D code?

But why? The anethesiologist doesn't know if it is an adenomatous or not. I guess that is what isn't making sense to me.

If the doc removes a polyp I wait for the path and that is what I code the dx as.
