Is anyone familiar with this procedure? The doctor dictates that he makes an incision on each side of the vertebral body to prepare for implant. Do you code 22612 twice, once for each incision, or just once, since he is technically only fusing one level? HELPPPPPP
 
I am not a spine specialist..but will offer my two cents.

Since it is only one level, I would only code it once regardless of the number of incisions because the procedure code specifically states "per level".
