What modifier can be used if we have two physicians (same tax ID number) seeing the same patient on the same day but each seeing for a different diagnosis?

I've been unable to find a modifier that fits the bill. I would probably just submit two separeate claims to the carrier and send along the documentation.

Same specialty?

Are they in the same specialty? If yes, then you can only bill one E/M code ... use all the documentation to support the level. You should have a protocol in place to cover this. (In our practice, whoever sees the patient first gets all the code. In other practices it might be whichever physician documents the highest level of service.)

If they are of DIFFERENT specialties (for example, internist and orthopedic surgeon), you don't need a modifier at all. Each physician's service is coded as per his/her documentation.

Hope that helps.
