I an concerned about unbundling. Is it possible to bill for all of these CPT codes for a cerebral angiogram?

No, that way to many codes. If you did a diagnostic exam of the bilateral internal carotids, bilateral vertebrals- selected, and external carotids- bilaterally, you would have six codes. They would be 36224 x 2 - selective internal carotids (can use rt or left or -50 for the modifiers, depending on what the payers want.), 36226 for the selective vertebrals, and 36227 for the selective external carotids. This includes all catheter work and imaging.
