One of our providers mixed up the medication to give the injection, but the parent refused it at the last minute. I have the diagnosis code for the refusal, but can we bill for the medication since it was already mixed up and we had to waste it since the parent refused?

I don't think you can charge the patient if they didn't receive the vaccine. Because if you charge them then you have to document lot numbers etc and it will look like it was given. If it's private ins, you will just have to eat it. If it's VFC, need to call the state and let them know what happen. Sorry that happen.

This is what the incomplete procedure modifier code is for. Addend -52 or -53 to the codes for the vaccination.

Your clinic did the work of education, drawing up, and lost the vaccination. Your clinic deserves reimbursement.
