I have a chart that is coded a 99253.

The consult page is signed by both docs.
The reason for consult is blank.

On the H&P, the reason for consultation is: Management of underlying medical condition.

I'm looking at it as a 99232. I have an extended HPI and ROS; a complete history and a moderate MDM.

I agree with you, this is a request for co-management so the consult codes would not be appropriate.
