I bill occpational medicine wc Indiana patient came in corneal abrasion also rec'd a tetanus at that time next day return visit had an allergic reaction to tetanus corneal abrasion resolved changing the dx for his return to
995.27 Allergic reaction properly admin
E948.9 tetanus w/diph (thats what we use) therapeutic use correct substance was given
729.81 Swelling in limb
Is there anything else i should add?

If the reason for the follow-up visit was to check on the status of the corneal abrasion, then it needs to be captured as well.

I would use DX code 999.5 instead of 995.27 as it is a reaction to an immunization.

I believe you have all the correct codes, but if the swelling was part of the allergic reaction, I would put that between the ICD 900 series and the E-Code.
