What is the difference between Z01.419 and Z11.4? If a client (for pathology) gives us the narrative "routine pap exam," which do they mean?
 
Z01.419 is the diagnosis the clinic could use for the encounter for the routine gyn exam without abnormal findings. If a pap was done and is coming to the lab for pathology, then the lab could use a screening type diagnosis code for the pap, since they aren't seeing the patient for the 'encounter'.

Z11.4 is also an encounter code but for screening for HIV, which would warrant something different from the "routine pap exam".
