Coding ALL, CLL, and Multiple Myelomoa. When would you consider the patient past remission coding and use the history of codes?

Leukemia is coded in remission or historical when the doctor says that is the status. In my 20+ years experience, I have only coded a leukemia as history a handful of times. You, as a coder, cannot make that determination on your own.

In regards to multiple myeloma, I've never seen that discussed as in remission or historical. Once the patient is diagnosed, it's coded active until the patient passes away.
