Hello, I am posting this for someone who is not on this forum. I don't know what it is.

"One of the docs filled out a medical necessity and prior approval form. She is asking if I can bill for this. I thought I could but I don't know what code to use"

the CPT is 99080 for special reports, the ICD-9 is a code I think in V65 category I do not have my book on me at theis moment but it is an administrative code for forms.
