Hi, hypertension is not a symptom, its a diagnosis, cc means any compliants or symptoms from HTN like giddiness, headache, etc..... thats time we take location this not under location so its takeit under ROS.

its take cc:followup for HTN is correct sentence please check it.
 
I see! If the provider had documented something like: "Pt here for HTN. Pt has been hypertensive for 3 years and last week his BP was 200/120" Then I would have duration and severity, correct?
