I know we all just came off of a long holiday weekend and we are tryinig to get our minds back on work.

I am auditing a chart and the diagnoses are:

1. Chest pain, rule out acute myocardial infarction
2. New onset atrial fibrillation with rapid ventricular rate.
3. Alcohol abuse and dependence. Patient appears to be in early withdrawal.
4. Mild hyponatremia, probable due to hypovolemic hyponatriemia

It was coded: 427.31; 427.41; 786.50 and 305.00

I coded: 786.50; 427.31; 303.90 and 276.1

My question is -- why would you use 427.31 and 427.41 together?

One is "atrial", the other "ventricular".

I don't get an edit saying these two "fibrillation" codes should not be coded together, however I do not see ventricular fibrillation documented in your case.

Perhaps 785.0 for the rapid ventricular rate (??)
