has any one billed 25332 wrist arthroplasty with palmaris longus tendon graft with separate incision. Could I bill 25312 with a 59 modifier?:confused:

Per the operative report it is clearly dictated that the incision was done at the first dorsal compartment at radial ulnar joint and distal ulnar was excisied

An incision was made on the floor of the fifth dorsal compartment and the ulnar head was exposed. Dissection was carried down to sharply excise the ulnar styoid remnant fracture fragment since it was so displaced from the rest of the ulnar head. It was possible to remove this without distrubing the intact TFCC. A saw was used to remove the radial portion of the ulnar head leaving the ulnar side intact. The forearm was supinated and two small transverse incisions were made over the palmaris longus tendon on the flexor of the flexor side of the forearm. Scissor spreading dissection was done to mobilize the tendon and it was resected proximally at the musculotendinous junction. It was used as a graft.
