Anesthesia Coding:

Find the Right Code and Modifier for a Routine Screening Colonoscopy

Question: How do I code the anesthesia services involved for a 52-year-old healthy patient undergoing a scheduled routine screening colonoscopy?

Answer: First, you’ll need to identify the surgical code. The CPT® code for a diagnostic/screening colonoscopy is 45378 (Colonoscopy, flexible; diagnostic, including collection of specimen(s) by brushing or washing, when performed (separate procedure)).

Doctor proctologist holding endoscope during colonoscopy.

Then you’ll need to use crosswalk to find the appropriate anesthesia code. Looking at the American Society of Anesthesiologists’ (ASA’s) 2026 CROSSWALK®, lower intestinal endoscopic procedures map to 00811 (Anesthesia for lower intestinal endoscopic procedures, endoscope introduced distal to duodenum; not otherwise specified).

After you have the necessary code, you can assign a physical status modifier: The patient is healthy, making them a P1 (A normal healthy patient).

Your final code selection is: 00811-P1.
