The provider inserts a catheter through the skin (percutaneous approach) and guides it into a coronary artery bypass graft, which is a surgically created blood vessel that reroutes blood around a blocked coronary artery. They perform one or more procedures (angioplasty, atherectomy, and/or stent placement) to restore blood flow to the heart muscle. This code applies to revascularization of a single major coronary artery and/or its branches through or within the bypass graft.
