The provider inserts a catheter through the skin (percutaneous approach) and guides it into the heart?s arteries or a coronary artery bypass graft. They restore blood flow in a completely blocked vessel (chronic total occlusion) using an antegrade approach, meaning they advance wires and devices through the blockage in the forward, natural direction of blood flow. The provider may use angioplasty, stent placement, and/or atherectomy. This code applies to a single major coronary artery, branch, or bypass graft, and/or the coronary artery and branches that graft supplies (the subtended artery).
