I've posed this question before, but am still groping around for an appropriate way to code this.

The doctor is performing the above referenced procedure but without the tendon transplantation/transfer. He finishes the procedure by percutaneously placing a K-wire through the thumb metacarpal into the adjacent metacarpal.

I'm thinking of using 25447 alone (without 25310/26480) and attaching a mod.74.

Has anyone else encountered this situation and what could were used?

25447 is appropriate by itself. I would not attach the -74 on it because the procedure was completed.
