Wiki AC separation repair w/allograft reconstruction

Patient had arthroscopic repair of an acromioclavicular separation followed by a second arthroscopic repair for failed graft. This is the third arthroscopic repair and the cause of the most recent dislocation was an unspecified accident. My question is about DX coding. Would this be considered 831.04 or 996.49 for failed graft? Thanks.
 
I would use both dx codes and if you can find out how it happened, add the E code as well. If truly an accident that caused it, use the 996.49 as the
2ndary dx.
