There seems to be some discrepancy on whether a simple device (CPT 77332) should be billed for a cylinder, tandem, and/or ovoid for HDR gyn brachytherapy procedures for the hospital technical component. I'm seeking clarification and resources to determine appropriate coding.

The 2024 Radiation Oncology Navigator, specifically the section on Treatment Devices, states the following:
  • Simple block, standard bolus (guaze pads, resusable bolus bag, uncut superflab, water bag, water bath etc.), pre-made blocks, no special fabrication or customization required.
    • Brachytherapy applicators (cylinders, tandem and/or ovoids, templates)
      • Hospitals can bill brachytherapy applicators one time at first use for the course of treatment
ASTRO’s guidance is that the CPT codes for brachytherapy procedures/applicators specifically include reimbursement for the device and are not separately billable.
