I realized this thread is a bit old, but thought id answer
and im assuming you meant E11.621
Z48.00 should be at the bottom.
unless the specific Z code is an aftercare for a surgical procedure, or a resolved condition that still requires home health, that is going to be your focus of care, you would not code it primary.
Pretty much all your Z codes should be at the bottom.
You are taking up valuable real estate in the top six with dx that won't add to case mix.
