Wiki Code symptom or history code first?

Just curious. Does it matter if we code the history code or the symptom code first. No other diagnosis code is use.

You code the reason for the encounter first , or the diagnosis determined by the provider to be the reason for the encounter. Of course provided other code first rules are followed as well.
 
Symptom vs history code

We are seeing the patient for an MRI only. I was given a symptom and past history.

MRI Visit

You would code the reason for the MRI-which would in this case be the patient's symptom. Code the symptom as the RFV(admit dx) and the PDX
