I'm working on a chart:

CC: productive cough and shortness of breath.

HPI lists: SOB, productive cough, pleuritic chest pain.

Under the ROS it states:

Pt has some pleuritic chest pain, cough productive of yellowish sputum. No PND. No orthopnea. She denies any GI or GU symptoms. All other systems were reviewed and were negative.
 
I would credit, but each item only once, building up from chief complaint to HPI to ROS (which will be complete with the "All other systems negative")
