Only add modifier 91 if it is done more than once as pathology lab test. Other than that no modifier is needed. You can only put modifier 25 on Eval Mgnt CPT codes. I hope your diagnosis code is appropriate skin problem though for CPT 17311 and use it for the lab CPT 88342 too. I am guessing the dx would range in D21, L82.1 or D21.9 ,R22,D24 Etc.

Yes, you will need a modifier "if appropriate" to bill 88342 with 17311. I will state that it is always helpful to have a pathology report (without PHI) for a MOHs procedure with immunohistochemical stain(s) applied for assistance from anyone. I also will not use modifier 91 on any subsequent immunohistochemical procedures unless it is provider specific (such as Minnesota VA is one that requires that one).
