Pt treated with chemotherapy for advanced stage Hodgkins Lymphoma. After completion of chemo tx, pt had recurrence of disease to axillary lymph node. Pt subsequently treated with radiation and chemo therapy. My question is should I use a diagnosis code of 201.94
(lymph nodes axilla & upper limb) or 201.98 (lymph nodes multiple sites)? There's no mention of site in the original note for the original diagnosis, dx used 201.90.

IMO, multiple sites implies a more widespread disease process. I would use 201.94 for this case.
