Not sure, but you might try this:25360 osteotomy, ulna. It gives no other information, but when I go through 3M, this is what I get starting with excision going through bone, fragment, ulna.

is that all that is done. Simply opens and removes the fragment? Post the note?

yes that is all that was done....but here is the notes





a small incision was made directly over theanterior aspect. we were able to sharply dissect down through the subcutaneous tissue. the tissue was divided longitudinally in both the medicaland lateral plane. we were able to identify the nonunion fragment. under direct visualization the nonunion fragment was removed. complete removal was achieved. we copiously irrigated. we reapproximated the capusule. sutures were placed.
