Blue Cross denies a pre-op visit coded as: E/M with modifer 56 with a diagnosis code V72.83. Does anyone have a suggestion on how to code this pre-op visit?

the 56 modifier cannot be appended to the E&M, it must be appended to the procedure code planned to be performed. This is in the BCBS provider manual and V72.83 is the correct ICD-9 code with the dx code for the surgery listed second.
