I'm working on a chart where:

Dr. A discharged the patient on 11.20.08, pt did not go home that day. Pt was then admitted to hospice the next day 11.21.08 by the same doctor. What do I do?

I know I have to bill for the discharge but what about the next day?

Did Dr. A physically see and evaluate the patient on 11-21-08? If so, where was this service performed? Was the patient admitted to the Hospice program (where he may have gone home under hospice care) or was he admitted to an inpatient Hospice?

You code would depend on what services were rendered and where they were rendered.

Yes, Dr. A did indeed see the patient and discharged them.

The patient was admitted to an inpatient hospice program. He never left the hospital.
