Does anyone have a workaround for the Ophthalmoscopic examination of the optic discs (e.g. size, C/D ratio, appearance) and posterior segments (e.g., vessel changes, exudates, hemorrhages) in the comprehensive neuro exam?

Our providers are already checking the cranial nerve 2, so this seems redundant to me. It also requires dilation to examine properly, which is not always appropriate for every patient. It doesn't seem right that a provider who is seeing a patient with ALS is not able to bill a level 5 new patient visit, just because the opthalmoscopic exam isn't appropriate.

Please help.....we have a couple payers down-coding our level 5's to 3's for one point in the exam!!!
