What do you mean by "supervising doctor"?

When an assistant surgeon is present (and when the CPT code allows for an assist to be billed) - you code the appropriate CPT codes for the primary surgeon. You use the exact same codes with appropriate assistant surgeon modifier for the assistant surgeon.
