We have instances where a doctor picks an incorrect code (perhaps wrong laterality or not specific enough) in the patient's EMR. When we file the claim, we use the correct ICD10 code. Does the doctor have to do an addendum, noting the correct ICD10 code? We of course match the code to his dictation. Just wondering what other people are doing.
 
The medical notes should reflect what you coded. If the EMR record shows a problem on the right side but you billed for the left side, which was actually correct, then the notes need to be corrected in my opinion.
