If anyone could clear up an issue with the coding a GYN Annual exam, Z01.419/11 and a general annual exam, Z00.00 at the same time. In GYN/OB would one ever code both. Or are you suppose to code both or not. So confused on this. Is there written guidelines anywhere for proof on this issue?

Our office uses Z01.419 for the well woman with out abnormal findings and if something abnormal is found during the examination z01.411. We have had success and have not used the Z00.00 I hope that helps.

I work for family practice and use Z01.419 for just the pap smear. If we have a female patient that sees GYN for paps, we only use Z00.00. We haven't had a denial yet using ICD-10, but have a couple patient's whose GYN office used a V70.0 for the gynecological exam. When they came to us for the remainder of their physical, our claim got denied (using V70.0 as the only diagnosis code). You may want to keep that in mind as well. Hope it helps.
