I work in a practice where the providers do all of the coding before its signed off on, occasionally they look over small errors such as duplicate codes or incorrect codes. Do the providers have to make the addendum to correct those errors or am I as an office manager able to make that addendum after consulting with him on which is the correct code. Our office is extremely busy and the more we can take off of him as far as small things would be fantastic.
 
No addendum is necessary to correct a code. An addendum is only required to correct an error or add information to the medical record itself. If an incorrect code was chosen that does not accurately reflect the provider's documentation, then it can be corrected without making an addendum to the record itself.
