SUMMARY

By the end of this workshop, participants will clearly understand how the core components of Value-Based Care (VBC)—documentation, coding, Hierarchical Condition Categories (HCCs), quality, care coordination, and utilization management—work together to improve patient outcomes, reduce costs, enhance provider satisfaction, and strengthen performance in value-based contracts. The session emphasizes a patient-centered, team-based approach as the foundation of successful VBC.

Key Learning Points

  • CMS aims for all healthcare organizations to participate in Value-Based Care arrangements by 2030, and participants will explore the major VBC models driving this shift.
  • Value-Based Care is not about chasing metrics—it is about delivering high-quality, patient-centered care that naturally drives strong performance.
  • When teams focus on excellent care and execute their roles effectively, quality and financial metrics improve organically.
  • Accurate documentation and coding create a complete and reliable picture of patient complexity and care needs.
  • Effective care coordination improves outcomes while reducing unnecessary utilization and avoidable costs.
  • Quality initiatives, utilization management, documentation, and coding are interdependent and should not operate in silos.
  • Strong collaboration across clinical, coding, CDI, quality, and care management teams leads to a more efficient and aligned care delivery model.
  • Better alignment among stakeholders results in:
    • Improved patient outcomes
    • Higher patient satisfaction
    • Reduced clinician burden and abrasion
    • Enhanced care coordination
    • Greater cost efficiency
    • Stronger performance in value-based contracts
  • Understanding how documentation, coding, quality, utilization, and care coordination connect is essential for long-term VBC success.

Workshop Takeaway

Value‑Based Care is simply good care. When healthcare professionals deliver the right care, document it accurately, coordinate effectively, and collaborate across disciplines, both clinical outcomes and financial performance improve.
