Home Health Agency 5 Star and Value Based Outcomes are critical to understand and to have strategies with which to improve. This webinar will focus on these key outcomes, including how to understand the source, i.e. OASIS or Claims Based; and review each of these outcomes. We'll drill down on how to improve each Patient’s outcome thorough collaboration of care and a goal-oriented team. Action items will include education to engage staff and QAPI indicators and/or Performance improvement projects.

Learning Objectives/Agenda

• Understand how to select key problematic outcomes to focus on improving.
• Be able to develop a realistic action plan to improve low outcomes.
• Develop an education plan that will engage staff so they understand areas for improvement and can implement each.
• Provide care with an interdisciplinary team working together in order to coordinate the patient care in order to improve outcomes
• Develop quality indicators for low outcomes in order to improve and then sustain the outcomes.

Why is this topic important?

Learning how to improve outcomes is Not only for the quality of a Home Health patients, which is the most important, but also is a necessity with payment being tied to 5 Star and Value Based Outcomes. Agencies for example that are at 2.5 Stars out of 5 are frequently losing their referral sources. And the VBP performance will cause a financial penalty for low outcomes comparative to other agencies.

What’s the presenter's background/expertise on this topic?

Sharon has been a consultant since 2003 when CASPER Data based outcomes began in HH. Improving patient and AGENCY outcomes has been her focus. She also has been doing the full QAPI program since 1990. She has taught on improving outcomes for 3 decades and has worked with many agencies across the country to improve outcomes. She keeps up with all CMS 5 Star compare, CAHPs and VBP and utilizes much of this information in her teaching.

About The Author

Sharon Litwin RN, BSHS, MHA, HCS-D, COQ-S

Sharon has had an extensive career in home health, starting as a field nurse in 1985. Over the ensuing years, she held positions in California and Missouri as Director of Patient Services, Administrator, Regional Director, and Executive Director of a large hospital-based home services division.

In 2003, Sharon left the corporate world to create 5 Star Consultants, to utilize her knowledge in partnership with agencies in providing assistance and education to those in need. Since then, she has also been a CHAP and ACHC surveyor, performing Medicare deemed accreditation surveys for 10 years. 

In 2020, she sold the company to Healthcare Provider Solutions, and is now offering her expertise and knowledge to a limited number of clients in the industry. Sharon now lives in sunny Cape Coral, Florida, and is pleased to offer services through her new consulting agency, Litwin Home Health Consulting, LLC.
